Annual Report Submission Instructions 


There have been some significant changes to the process, so please read carefully through these instructions. Please remember that Annual Reports must be submitted through BoardEffect no later than January 15, 2018.

Process Overview

The Annual Report for the Year Ending December 31, 2017 may be accessed by clicking on the form in your BoardEffect workroom.  Please remember that if you do not complete the form in a single sitting, you must click SAVE at the bottom of the form or you will lose your work. Annual report forms will be available in BoardEffect by the end of the day, Monday, December 18.
  • Accredited Centers will complete ONE form.
  • Accredited System Centers will complete ONE form that incorporates all of the component sites in the narrative paragraphs. 
    • Accredited System Centers will also need to download the Component Site Completion Rates supplemental form that is located at the top of the Annual Report Form after you open it.  This form should be completed and uploaded to the Annual Report 2017 Supporting Documentation Folder in the workroom library.
  • Satellite Programs will complete the Annual Report Form for Satellites.  This document may be found at the top of the Annual Report Form after you open it and on the ACPE website.  Satellites should complete the form and submit it to their host centers for uploading into BoardEffect.
  • All programs that utilize video conferencing as a modality of instruction must also complete the Video Conferencing Utilization form that is located at the top of the Annual Report Form after you open it.  This form should be completed and uploaded to the Annual Report 2017 Supporting Documentation Folder in the workroom library.

Uploading Documents into BoardEffect

To upload the supplemental forms and all required agreements, 

A. Log into BoardEffect and locate your center’s workroom
B. Click on Library in the top menu bar.
C. Click on Other Files.
D. Click on Annual Report 2017 Supporting Documentation Folder.
E. Then, drag and drop your materials into the rectangular box or click upload to select files from your computer.

Go to the ACPE Website, hover over the Accreditation tab and click on Access BoardEffect. If you are not yet logged into the ACPE website, it will prompt you for your regular credentials.

Then, click on the orange Access BoardEffect button at the bottom of your screen.  Each center has a designated individual who can access BoardEffect. If you are the designated individual for your center but cannot access BoardEffect, please contact Kimberly Yates at kimberly@acpe.edu.

Once you have logged in, you will see the My BoardEffect Page.  From here, navigate to your center’s workroom by locating your accreditation area on the left hand column, click on it, then find your center’s name and click on it.  If you are not able to see your center, you may not be attached to the workroom. Please contact Kimberly Yates at kimberly@acpe.edu or Marc Medwed at marc@acpe.edu to fix this.

A copy of the instructions is located on the welcome screen of your workroom for your reference.

To begin filling out the Annual Report Form for the Year Ending December 31, 2017, please scroll down to the My Polls & Surveys section and click on the title.

Once inside the form, please note the files at the top of the screen. These are the supplemental forms mentioned in the instructions.  Clicking on the file name will automatically download a copy to your computer which you can save, complete, save again, and then upload into BoardEffect.



All questions marked with an asterisk (*) are required, so you must enter a value for each item.

Please remember that physical signatures are not required as part of the electronic form submission.

To upload the supplemental forms as well as ALL contracts/agreements, click on the Library icon at the top of your center’s workroom page.

 
Then, click on Other Files, located directly beneath the Library icon.

Then, click on the folder Annual Report 2017 Supporting Documentation.

To upload documents, you can drag and drop into the box, click on click for more options inside the box or click on the green Upload button. Once you see your files appear in the list, you are able to close the box.

If you need to replace a document, you can click on the down arrow that is located on the far most right side of the document, which will then open a box that allows you to choose delete.  Then, follow the instructions in #12 above to upload the new document.
